Integrate lifestyle screening assessments into health settings and clinical practice

Generate appropriate lifestyle and social prescriptions for health and wellbeing and the prevention and management of common chronic diseases

Adapt and integrate lifestyle and social assessment and prescriptions that are appropriate to varied geographical and cultural backgrounds

Work as part of interdisciplinary team to make patient assessments and provide social prescription recommendations
