Systematically plan safe, collaborative, effective, client centred, clinical care and self-management education for conditions within the identified scope of the Pharmacist.

Apply principles of accountable prescribing to the provision of integrated safe, accountable, and culturally sensitive patient care, utilizing a collaborative approach with patients, their carers, and other healthcare professionals, underpinned by the biopsychosocial model of medical assessment.

Reflect on current skills, knowledge and attitudes, and plan ongoing professional development.

Identify, evaluate, and apply established evidence and approved clinical protocols in the provision of care and education for listed within a Pharmacists expanding scope of practice.

Conduct an appropriate consultation and/or medical history, physical examination, and order and interpret appropriate pathology and investigations, to develop a working diagnosis within scope or refer out-of-scope patient presentations.

Document and communicate treatment options, agreed plan, and monitored progress with patient, carers, and patients’ other health care provider.
